Move the camera with the right stick. Access the grid icons with the face buttons and R3. Pressing the left stick up activates the top row of hotkeys, pressing it down activates the bottom row, and leaving it neutral gives access to the middle row.

Just copy the above configuration into a text file, change the IN_GAME_SENS to match your in-game settings, and then drag that file into JoyShockMapper and hit Enter to use it right away.
